I do not know if the source code link problem is still an issue for in-source. I am aware that the out-of-source sphinx does not properly find source code. Here's a snippet of the warning messages generated when working in an out-of-source situation:

../../common/bcda/checkbeam.mac:1: WARNING: download file not readable: /home/oxygen/JKIRCHMAN/bcda/svn/spec/common/sphinx/source/../../common/bcda/checkbeam.mac

../../common/aps/do.mac:1: WARNING: download file not readable: /home/oxygen/JKIRCHMAN/bcda/svn/spec/common/sphinx/source/../../common/aps/do.mac

../../common/bcda/epics_misc.mac:1: WARNING: download file not readable: /home/oxygen/JKIRCHMAN/bcda/svn/spec/common/sphinx/source/../../common/bcda/epics_misc.mac

I believe that the out-of-source is not compensating for the additional directory level that has been added due to it being an out-of-source doc. As you can see, the file is not found because the level mismatch of directories. If another "../" is added to the directory path, the source file is found.
